Job Title: Care Coordinator

Position is in South Woodham Ferrers, Essex and relocation is required. Own transport is also essential.

This Care Coordinator position is a full-time position that requires someone with experience within the care sector, as well as knowledge of scheduling software (Birdie system, People Planner, Rota)

Responsibilities:
  • · Ensure that the Quality of Service Provision meets with CQC guidelines and expectations.
  • · Care Coordinator ensures that Regulatory Requirements are met.· To implement a high-quality service through effective matching of service user needs with suitably qualified and experienced care workers.· Care Coordinator liaise with the registered manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion.· Ensure working patterns are efficient and planned logically.· Care Coordinators effectively prioritise tasks to meet unexpected emergencies and changes in care packages.· Ability to maintain clear and accurate written records and follow statutory reporting procedures· Care Coordination ability to communicate clearly and build positive working relationships with clients, their families, fellow staff and other health and social care professionals.
Additional responsibilities:
  • · Receive and handle complaints about services from service users or others;
  • · Carry out on-call coordinating duties outside normal office hours as agreed.· Carry out risk assessment· Write and review client care plans· Supervise care worker
The ideal candidate will have:
  •  Full UK driving license
  • · A strong background in Health and Social Care· A minimum NVQ Level 3 qualification or above· A minimum of 2 year Care Coordinator experience in the domiciliary care sector· Strong IT and logistical skills· Exceptional verbal and communication skill is essential· Organisational skills· Good geographical knowledge
